REQUEST FOR ASSISTANCE

"SAMPLE" FORM

The above Request for Assistance Form may be used by customers to help them request missions.  It outlines the information needed by the CAP National Operations Center (NOC) in order for the CAP NOC staff to coordinate mission approval.  CAPR 60-3 allows the NOC to act on verbal requests for CAP assistance in an emergency, but all requests for CAP support must be submitted in writing; even verbal requests must be followed up in writing.
